PASTA WILL NOT BE AVAILABLE UNLESS ORDERED IN ADVANCE.

Hot Dog Order Form
We will be serving hot dogs at lunch time on the first Monday of every month unless otherwise noted.
Payment is due the Monday before.
Hot dogs will not be available unless ordered in advance.

Chicken Nugget Order Form
Chicken Nuggets will be offered for lunch on Select Mondays each month.
Chicken Nuggets will not be available unless ordered in advance.
Orders are due on Monday, the week before Chicken Nugget Day.

Extended Day Registration Form
An affordable extended day program is directed by staff members and offers children a safe environment with scheduled times for snack, indoor and outdoor activities, and supervised homework.. This program is available Monday through Friday from 2:30 p.m. to 6:00 p.m.

Early Drop Off Registration Form
The Early Drop Off Program is open to students Pre-K through eighth grade. Students should be dropped off at the main entrance of the school at 7:30am and are supervised by a faculty member in the Early Drop Off room. At 8:00 Pre-K students are brought to their classroom, and students K-8 are walked to their line in the schoolyard. The cost of the program is $2.00 per day. The program can be used daily or whenever the need arises.

Student Absent Notes
Parents are expected to call school before 9am to report an absence. A written excuse is to be given to the teacher on the day the child returns to school. This printed form is available for your use.
